Gangrenous cholecystitis complicated with haemobilia

Abstract

A 32-year-old patient presented with icterus, elevation of liver enzymes, clinical and laboratory manifestations of cholestasis and intra-abdominal echographic findings of thickening of the vesicular wall. The clinical evolution was torpid, with abdominal distention, hiccups and acute renal failure accompanied by metabolic acidosis, requiring renal purifying treatment. In an attempt to guarantee deep venous access using a catheter, a hematoma of the neck occurred and a left pneumothorax was induced, requiring a minimum mean pleurostomy and connection to the Overholt equipment. Subsequently, the patient had a cardiorespiratory arrest with electrical activity, without a pulse, and died after failed resuscitation, at 14 days of hospital stay. At the necropsy, an acute gangrenous cholecystitis without stones was found, complicated by haemobilia and obstruction of the biliary tract by an intracoledocyte clot and development of ascending cholangitis, all of which led to the death of the patient due to hepatorenal syndrome.
