Anesthesia in patients with von Willebrand disease: Case report

Introduction: Von Willebrand disease is the most common inherited bleeding disorder and is caused by a deficiency or dysfunction of von Willebrand factor, which mediates platelet adhesion and aggregation at sites of vascular injury and also transports and stabilizes coagulation factor VIII. Patients present with mucocutaneous bleeding, including ecchymosis, hematomas, prolonged or excessive bleeding after minor cuts, epistaxis, gastrointestinal bleeding, menorrhagia, and exhibit a higher risk of bleeding after surgery, invasive procedures, dental extractions, traumatic injuries, or childbirth.
Patient information: Patient diagnosed with lower gastrointestinal bleeding who underwent colonoscopic polypectomy and was diagnosed with type 1 gastrointestinal bleeding.
Conclusions: The main considerations are related to the high risk of perioperative bleeding, preoperative administration of desmopressin, administration of tranexamic acid, impeccable surgical hemostasis, and the availability of blood and platelet concentrates.
