Abstract

Introduction: The prognosis and management of the difficult airway are of vital importance in anesthesiology. Objective: To assess the usefulness of the Wilson scale in predicting difficult endotracheal intubation.
Method:
An observational, descriptive, cross-sectional study was conducted at the “Celestino Hernández Robau” University Hospital in Santa Clara, Argentina. Of a population of 2,230, 630 patients who underwent elective or emergency surgical treatment with endotracheal general anesthesia between June 2021 and May 2023 were studied. The degree of intubation was assessed using individual variables and the Wilson and Lehane-Cormack scales.
Results:
The frequency of difficult intubation was 1.75%. Predictive tests for difficult intubation generally showed high percentages of negative predictive value and specificity, and low percentages of positive predictive value and sensitivity, indicating they were better at ruling out difficult intubation than at correctly identifying it. The Wilson scale revealed positive predictive values of 7.69% and negative predictive values of 99.28%, sensitivity of 63.64%, and specificity of 86.43%. The Lehane-Cormack scale showed the best results.
Conclusions:
The indicators alone were not good predictors of difficult intubation. The Wilson scale was better at identifying patients with an easy airway and showed evaluative superiority over the individual predictive tests, but it did not demonstrate favorable values for predicting difficult endotracheal intubation.
