Incidence of supracondylar fractures of the elbow in pediatrics: percutaneous lateral fixation

Introduction: supracondylar fractures of the humerus are frequent in children, include 50 to 70% of the injuries of the elbow and occupy the first place of the fractures that require surgical intervention in the pediatric age. Objective: to determine the incidence, the results in the treatment and the complications of the patients with supracondylar fracture of the displaced humerus. Method: an observational, descriptive retrospective cross-sectional study was carried out in children under 15 years of age with diagnosis of displaced distal humerus supracondylar fractures who were assisted at “José Luis Miranda” Hospital from January 2016 to January 2017 and who were treated with lateral percutaneous fixation; the incidences, the results of the treatment and its complications were determined. The sample consisted of 32 children. Results: the main traumatic cause were hyperextension falls in the elbow (94% were closed); the Gartland classification was used for the assessment of fracture displacement and the results were studied, both from a clinical and radiological point of view, and the Flynn criteria were followed (83% corresponded to a Gartland type III); all had closed reduction with smooth percutaneous nails of lateral and parallel disposition; After surgery, median nerve involvement (3,1%), joint stiffness (6,25%) and infection (3,1%) were found, and excellent and good results were demonstrated in 96,8% of patients. Conclusions: closed reduction and percutaneous fixation with parallel nails are recommended for displaced and unstable supracondylar humerus fractures.Downloads
